It came out as a digital single on 17 December 2015, produced by Bowie and Visconti at the Magic Shop and Human Worldwide. Bowie sang and played guitar, McCaslin played saxophone, and Lefebvre, Guiliana, Monder and Lindner made up the band. Visconti said the lyrics and the video were a self-epitaph. The title is shared by the off-Broadway musical Lazarus, and Bowie never played the song live. Michael C. Hall sang it on The Late Show with Stephen Colbert on 17 December 2015.
Johan Renck directed the video, which went online on 7 January 2016, three days before Bowie died. It shows him on a deathbed with bandaged eyes and buttons over them, and ends with him retreating into a wardrobe. It received three MTV Video Music Award nominations in 2016.
In the week after his death it reached number 40 on the US Billboard Hot 100, his first top 40 appearance there in over 28 years. It also reached number three on the Hot Rock and Alternative Songs chart and number 45 in the UK. G-Eazy covered it in 2020.
