XTRMNTR takes its title from a disemvoweling of Exterminator. Creation released it in the UK on 31 January 2000 and Astralwerks in the US on 2 May 2000. It reached number 3 in the UK and has a Metacritic score of 90 from 16 reviews. After the hedonism of earlier records the band took a more political stance, attacking government, police and multinational corporations. The sound is noisy and harsh, with electronic textures reminiscent of industrial music. Gillespie said the band wanted to convey what it was like to be in Britain at that time.

Mani shared songwriting credits for the first time. Kevin Shields of My Bloody Valentine helped produce the album, played on it and was seen as a semi-permanent member around its release, and the Chemical Brothers and Bernard Sumner of New Order contributed. Other producers included David Holmes, Jagz Kooner, Hugo Nicolson, Dan the Automator and Brendan Lynch. Swastika Eyes appears twice, in the Jagz Kooner mix and the Chemical Brothers mix. The album is the last full-length release on Creation, and the single Accelerator was the label's final single.

NME rated it the second best album of 2000 and gave it Best Album at the 2001 NME Carling Awards, and Uncut also ranked it second. Q put it among the 50 heaviest albums of all time in 2001, and in 2019 it ranked 65th on The Guardian's list of the 100 best albums of the 21st century.
