Delusion is the soundtrack to Carl Colpaert's neo-noir of the same name, and the first score that Adamson wrote for an actual film. He has said that it came out of Moss Side Story: he travelled to Hollywood about nine months after that album to work on it. Adamson and Paul Kendall produced the 17 tracks, with Seamus Beaghen on keyboards and Tito Heradia on Spanish guitar.

Trouser Press found a playful side that his earlier records mostly lacked, with menacing organ, flamenco guitar and spaghetti western sass that suited the film's double dealings. The album also holds the recording debut of Miranda Sex Garden, whom Adamson discovered, on Il Solitario, and a chugging version of These Boots Are Made for Walking sung by Anita Lane.
