Her first greatest hits album gathered fifteen singles, from Holiday (1983) to Vogue (1990), plus two new songs: Justify My Love, made with Lenny Kravitz, and Rescue Me, made with Shep Pettibone. Pettibone reworked the older material over a month and a half using QSound, making it the first album to use the process. The booklet was dedicated to The Pope, my divine inspiration, which was her brother Christopher Ciccone, whose nickname is The Pope. Her working title was Ultra Madonna.

The album reached number two on the Billboard 200 and topped the charts in Argentina, Australia, Canada, Finland, Ireland and the United Kingdom. It is certified 11 times platinum in the United States and has sold more than 30 million copies, the best-selling compilation by a solo artist. Erlewine of AllMusic noted that the QSound work meant the hits were not in their correct versions, while Robert Christgau called it the greatest album of her mortal life.
