Max Cavalera wrote the lyrics. He has said the idea came from the phrase Protest and survive, refuse and resist, painted on the jacket of a man he saw on the New York subway, which he wrote down at the hotel. He said a grinding riff in his head reminded him of a riot, and called the song anti-police. Discharge and GBH partly inspired the lyrics. It begins with the heartbeat of his then-unborn son Zyon, recorded at a clinic on a MiniDisc, and ends with a roar of about 20 seconds. The drum salvo at the start was played by Igor Cavalera on a smaller piccolo snare.
The band recorded it at Rockfield in South Wales with Andy Wallace. The single sleeve shows a protester confronting riot police, and the video cuts in footage of rioting. The song went to 26 on VH1's 40 Greatest Metal Songs. In 2010 Soulfly, Max's later band, recorded a version for Omen with Zyon on drums, aged 17.
